Registration:
Those who wish to attend the Congress should complete the Registration Forms. Each registrant must complete a separate form. Registrations can be made via the Congress website online registration. If you chose manual registration by fax or post, please ensure it is clearly written using BLOCK CAPITALS.

- Certificate of Attendance.
Please note that the registration fee does not guarantee you a seat at any particular scientific session. To avoid any possible disappointment we recommend you to arrive early. Accompanying persons Registration includes :

In addition to the above, accompanying persons may attend all social events and tours at their own expense.

Acknowledgement:
Your registration and payment will be acknowledged with confirmation number and the list of the requirements as per your registration form. Registration will not be
processed or confirmed if payment is incomplete.
Please ensure that you keep your "Bar Code" registration number safe and bring with you to the congress to receive your delegate pack. Â
Change and Cancellation Policy:
Any changes or cancellations must be received in full and in writing. Refunds will be issued according to the reimbursement rules. Please allow a minimum of 8 weeks for processing any refund. Refunds are based on the total transactions for all registration fees, courses, tours, and social events. Participants are advised to keep a copy of the registration form.
Cancellation Dates:
Before 10th June 2011 – 90% reimbursement
Between 10th June and 10th Aug.2011 – 80% reimbursement
After 10th Aug 2011: no reimbursement
Visa
Visa on arrival for citizens of GCC, European Union, USA, Canada, Australia, New Zealand, Japan, Brunei, Hong Kong, Singapore, South Korea and Malaysia. Other nationalities please contact the organizers for further assistance.
